New Patients

Your first visit to Aurora Foot & Ankle Clinic establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history. Below you'll find all the practical information you need and can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

STEP 1: Making an Appointment
Visit our Office Info page to view office hours and location details. You can call our office directly to schedule an appointment at (604) 248-8985 .

STEP 2: Patient Forms

Please print and fill out these forms prior to your first visit:

Completed forms can also be saved and emailed back to reception at the following email address: [email protected]

In order to view or print these forms you will need Adobe Acrobat Reader installed.

STEP 3: Other Items to Bring
Please also bring the following to your appointment:

  • Any recent medical reports such as xray, ultrasound, CT or MRI's
  • the shoes you most commonly wear
  • Any previous orthotics or other foot related devices
  • Some form of payment (see note below). We accept: Cash, Debit, Visa, Mastercard and American Express

STEP 4: Payment & Private Medical Insurance
PODIATRY IS NOT A COVERED SERVICE BY MSP OF BC, therefore all BC Podiatrists are private pay.

We bill patients directly and provide an invoice for submission to private insurance (extended medical coverage) if applicable. Our rates are the same regardless of insurance coverage or lack thereof. In most cases, patients with extended or private medical insurance have coverage for a variety of podiatry services. We recommend contacting your insurance provider in advance for more specific coverage details.
